📍 Ohio | 💲 $84k-$85k | 🕒 Full-time | Flexible shifts + on-call
Work 4-5 days a week 5pm - 8pm and every other weekend
triaging calls after hours and going out for symptom management as well as death visits and others that arise
Get paid for 40 hours even if you work less!
We’re hiring experienced Registered Nurse (RN) Case Managers to provide compassionate care in patients’ homes. You’ll assess needs, create and manage care plans, educate families, and ensure quality care that meets state and federal standards.
Requirements:
Current Ohio RN license
1+ year acute care or med-surg experience (home health preferred)
Valid driver’s license & reliable transportation
